医患关系中的“歧视”存在吗？——一项关于“行”与“形”的调查实验

摘要： 我国当下医患关系备受关注,医护人员与患者之间的摩擦时有发生,其中农村患者可能因为刻板印象受到医护人员的歧视,这也加深了医患的误解和矛盾。运用行为公共管理的理论视角和调查实验的研究方法,探究医患纠纷中的患者行为方式与身份特征的影响,可以为应对医患等社会群体矛盾提供一个新的视角。实验1分析了医护人员的判断,实验结果表明:行为方面,患者选择暴力的掌掴方式时,医护人员会降低对患者的同情心以及对事件的己方责任认定;身份方面,医护人员并不存在对农村患者降低其同情心并减少己方责任认定的情况。实验2分析了一般社会群众的判断,实验结果表明,城市人群在医患冲突时对农村人群没有明显的歧视;但农村人群反而存在一定的自身歧视。对于改善医患关系,研究建议患者应当意识到伤医得不偿失,此外政府应当落实乡村振兴的成果转化,提高农村人群的群体认同感,消弭社会中对医护工作者、城市群体会歧视农村群体的误解。

Abstract: The doctor-patient relationship in China has drawn much attention as friction between medical staff and patients arises from time to time and rural patients to some extend are discriminated against by some medical staff due to stereotype, which further deepens the misunderstanding and contradiction. This paper, by employing the behavioral public management, a theoretical perspective, and the research method of experimental investigation, explores the influence of patient behavior and identity characteristics on doctor-patient disputes. Experiment 1 analyzed the judgment of medical staff and the results show: medical staff, in terms of behavior, if patients choose violent slapping, will reduce their sympathy for patients and their own responsibility for the event; while for rural patients, in terms of their identity, medical staff do not bear the same attitude. Experiment 2 analyzed the judgment of the general public and the results show: urban residents bear no obvious bias against rural patients in the doctor-patient conflict while the latter bears some discrimination against themselves. This study, for improving such a relationship, suggests: 1) the gains from violence, patients should be aware, outweigh the losses; 2) the government should implement the transformation of the achievements of rural revitalization, thus improving the group identity of rural residents, and eliminating the misunderstanding that medical workers and urban residents bear against rural groups. The research provides a new perspective for analyzing the role of behavior pattern and identity characteristics in public conflict and dealing with the contradictions of social groups such as doctors and patients.
